About This Role

Sunrise Senior Living is hiring Geriatric Nurse Practitioners to deliver on-site clinical care to residents across its assisted living and memory care communities. Role Context

Sunrise NPs are embedded within the community and build long-term relationships with residents and their families. You will carry a panel of approximately 80–120 residents per community, visiting each on a scheduled basis and responding to acute changes in condition. This model reduces emergency room utilization and improves quality of life for frail, elderly residents.

Key Responsibilities

• Conduct comprehensive geriatric assessments and develop individualized care plans
• Manage chronic conditions including dementia, Parkinson's, CHF, and COPD
• Respond to acute clinical changes and determine appropriate level of care
• Manage polypharmacy and conduct medication reconciliation reviews
• Conduct advanced care planning discussions with residents and families
• Coordinate with hospice, home health, and specialty providers as needed

Requirements & Skills

• FNP-BC, AGNP-C, or GNP-BC certification
• Active state NP license with prescriptive authority
• DEA registration required
• Experience with elderly populations and dementia care preferred
• Comfort with end-of-life and advance directive conversations
• Strong communication with families and interdisciplinary staff
