Role Context

Oak Street clinics are built around smaller panel sizes — typically 400–600 patients per provider — intentionally designed for thorough, unhurried visits with elderly patients. NPs work alongside care teams that include health coaches, social workers, and care managers, with a shared goal of keeping patients healthy outside the hospital.

Key Responsibilities

• Manage a panel of Medicare patients with complex, multi-morbid conditions
• Conduct comprehensive annual wellness visits and chronic disease reviews
• Prescribe and adjust medications for conditions including CHF, COPD, and diabetes
• Collaborate with care navigators on post-discharge follow-up within 48–72 hours
• Lead advance care planning conversations and document goals of care
• Participate in weekly panel management huddles with the interdisciplinary team

Requirements & Skills

• FNP-BC, AGNP-C, or ACNP-BC certification preferred
• Active state NP license and DEA registration
• 1+ years of primary care or geriatrics NP experience preferred
• Comfort with value-based care metrics (HEDIS, HCC coding)
• Strong geriatric assessment and polypharmacy management skills
• Empathetic communication style suited to elderly and vulnerable patients
